First Appointments
For the initial foot care appointment, the client is to pay up front. If a client has private insurance, then a medical receipt for the service will be written after the appointment so the customer can send it to his or her insurance company. For those clients who have direct billing with DVA, the appropriate paperwork will be completed and sent on behalf of the client. Please bring your K# with you. For clients requiring compression garments, a signed Doctor’s prescription must accompany the client to the appointment before any garments are ordered. Prescription should include what level of compression is required, what part of the body the garment is for, and how many.
Cancellation Policy
Please be advised that we require 24 hours advanced notice if you are unable to make your scheduled appointment. Any cancellation less than 24 hours notice will be charged a $25.00 penalty fee. Thank you for your understanding.
Discount Policy
Stride Well will recognize those clients who will refer three new clients by giving them a 20% discount for each time three new clients are referred. For those customers who purchase two gift certificates for any service offered at Stride Well, a 20% discount will be honoured on the next appointment.
Forms of Payment
Stride Well will accept cash or cheque form for foot care service. Compression garments can be paid by cash, cheque, or Visa.
